Martial arts has risks like any other physical activity, the enrichment program is about learning to respect other people. It is important to listen to your body,and respect it.

As parent/guardian, I give my consent for my child(ren) to participate in Enshin's Enrichment Program. I recognize the risks of injury common to any martial arts classes or program. I hereby waive
and release Enshin, and any Officers, Employees, Agents and Assigns from any and against all claims of
liability, including accidents or injury while on or about their premises.
To the best of my knowledge, my child is in good physical condition and I have no knowledge of any physical condition, injury,
or illness whatsoever which would place my child at risk to participate in a martial arts class.
I understand that during a class, the Instructor may touch my child to adjust him or her in practice to maintain alignment, safety or support. I understand that for every minute I am late I will pay the instructor $1 a minute up to 10 minutes.
